Have a colonoscopy at a designated facility - $500 REWARD

The cost of colonoscopies can vary drastically between facilities. You can help keep these costs down by using one of the facilities identified as having reasonable costs, and earn a $500 reward for doing so.

This incentive program is available to every Medical Practice Consortium subscriber and covered dependent. Limited to one reward per subscriber or dependent per calendar year.​

To take advantage of this offer:

​

1. Have a screening or diagnostic colonoscopy at one of the designated facilities (see complete list on Incentive Request Form.

2. Complete a Colonoscopy Incentive Request Form and scan and email it to ismaia@ismanet.org; mail it to ISMA Insurance Agency, 322 Canal Walk, Indianapolis, IN, 46202; or fax it to (317) 261-2238.

Allow 60-90 days for your service to be verified and for an incentive check to be mailed to your home address.
